When referring a client, it is extremely helpful to gather and forward the following information. Providing this information ahead of time dramatically speeds up the evaluation process.

• Date of court hearing
• Date evaluation needed
• Any pending court dates
• Current court order for custody
• Interim order for custody
• Prior court order for custody
• Order for contempt
• Petitions for modification
• Pretrial memorandums
• Client’s criminal record
• Prior or current evaluations

Any pleadings relative to current action such as:

• Orders for Special Relief
• Orders directing that the client undergo a Threat of Harm evaluation
• Any information from Children, Youth, and Families (if applicable.)
• Contact information for parties involved with children
